If you are a traveler from a visa-eligible country going to Singapore, then you must fill out Form 14 A, also known as a Singapore Entry Visa Application form.
Please note embassy officials require it. It will also be requested by the Immigration and Checkpoints Authority (ICA) when applying for your online visa via SAVE (Submission of Application for Visa Electronically).
To obtain a Singapore visa, you must ensure that Form 14 A is filled in legibly. Please note that filling an incomplete Form 14 A for Singapore will lead to a delay.
We suggest writing your email id and telephone number on the top right-hand corners of the Singapore 14 A Form. This way, the embassy can contact you if the immigration officials wish to establish further correspondence.
Currently, only citizens of visa-eligible countries traveling to Singapore are required to fill out Form 14 A.
Form 14A will not be applicable to U.S. citizens as they are not listed in the visa-required countries list. Since U.S international passport holders are not required to apply for a visa prior entering to Singapore, an entry visa application will not be relevant to them.

If you are an American traveler who is already in Singapore and wishes to extend your stay further, the application for a social visit pass extension can be done through this ICA official website.
The form clearly states "Form 14 A" at the top of the application. Below it, the words " Immigration Act 1959, Section 55 [1]" are mentioned. On the top left corner is the heading "Notes," where you are advised to check (place a tick mark) where appropriate in the form.
In the right-hand corner, you will find a section for affixing photographs where it is mentioned, "Affix a recent photograph here."
Although it is not mentioned on the form, Singapore Visa requirements list that a photograph should be taken within three months. It is followed by the heading in the center, "Application for Entry Visa."
In the first section, you are supposed to write your full name as mentioned on the travel document. There are adequate boxes to fill out each letter of your name.
Singapore’s Entry Visa Application form expects you to declare your alternate name or "Alias."The next section is the Date of Birth, which should be filled in as DD-MM-YYYY format. This is followed by the declaration of gender, where you should fill out Male or Female.
In the next section, demarcated by a line, you must declare your Marital Status. While filling out Form 14 A, we had to choose from the following options:

If you have a spouse, who is Singaporean, you must check the relevant box ("Singapore Citizen") and fill out the respective NRIC number.
The second option is if the spouse is a "Singapore Permanent Resident," followed by the NRIC number box.
If you choose the option "others," you need to specify it on the form.

In the field that follows, you must write down the travel document number. This is followed by the issuance and expiration date of your travel document in DD-MM-YYYY (date, month, and year format).
Applicants can then declare their country or place of issue to complete the first part of the form.The last section of Page 1 of the form is where Chinese nationals are required to fill in their PRC ID number.
At the bottom of the page, in the left-hand corner, it is written IMM 14 A. In the center, the numbering "1," and below it are the words "This Form is Issued Free."
In this section, you need to fill out the address, place of origin, and residence in this section. It is further subdivided into the following four fields:
In the next line, you can write down the full address. Please note that the address on your passport should match the address on the form. Many travelers have reported on forums that their entry visa form was rejected because the address did not match.
This marks the end of Part 1 of the form.
In this section, those of you planning to visit Singapore must mention your email addresses, contact numbers, and occupations. The next segment gives you choices related to your education.
While filling out Form 14 A to obtain a Singapore entry visa, we had to mention our highest academic and professional qualifications. We had to choose among the following options:

We chose the option more than thirty days and declared it to be a social visit.

This marks the end of the second page. At the bottom left, it is mentioned IMM 14A. It is written "2" at the center of the form, followed by the disclaimer "This Form is Issued Free."
In this segment, if you are planning to go to Singapore, you must mention details related to your address in Singapore.
You need to declare where you will stay in Singapore and need to put a tick mark on the following options, whichever applies to your case:
After selecting any of the above options, you will further need to provide a block or house number, floor number, unit number, postal code, street name, contact number, and building name.
In the next section, we had to answer if we had stayed in any other country or place other than ourcountry or place of origin for one year or more during the last five years. Our answer was "No."
If your answer is "Yes," you must furnish details in the next section. The three columns- Country/Place, Address, and Period of Stay (further subdivided into two columns, " From " and "To.") need to be filled out by you.
In the other half of the Singapore entry visa application form, on the third page, you must mention the details of your traveling companions. Please note that this section is only for an applicant who is 12 years old or less at the time of application. No details are required if an airline representative accompanies the applicant.

This marks the end of Page 3. The left-hand corner states "IMM 14 A"; at the center, it states Page 3, followed by the line " This Form is Issued for Free."
The last page of your Singapore Entry Visa Application form consists of part 3, part 4, and part 5.
In this segment, those of you planning to go to Singapore need to fill out details of your Local contact or Company and Hotels in Singapore.
Depending on whether you have a local contact, staying at your company accommodation or hotel premises, you need to fill out the following fields:
In the next line, travelers need to share your contact number and email address of the contact, company, or hotel.
You must answer the following questions in a "Yes" or "No" format:
If the answer to any of the above questions is " Yes," you need to furnish the details in the next few lines allocated for it on the form.
In the last section of the Singapore Entry Visa Application form, also known as Form 14 A, you need to attest that the information you have shared on the form is true, accurate, and complete to the best of your knowledge and belief.
You also need to understand that declaring any false, misleading information will lead to prosecution, and all facilities granted under the Singapore Entry Visa Application form will be withdrawn.
You must undertake not to misuse any controlled drug and are not allowed to participate in any political activity. In addition, you are advised not to participate in any activity that would make them undesirable or prohibit your entry under the Immigration Act 1959.
You need to comply with the provisions of the Immigration Act 1959, and you must follow any regulation that comes under it while you are in Singapore.
You cannot be a part of any criminal offense or indulge in any activity inconsistent with the issuance of the Singapore pass. You further need to undertake that you will not engage in any occupation, employment, or business without a valid work pass issued under the Employment of Foreign Manpower Act 1990.
Thereon, you need to undertake that you will not overstay or remain illegally in Singapore as it is a serious offense. If convicted, the punishment is caning and mandatory imprisonment.
The immigration pass of you and the family member can be canceled if the Controller of Immigration becomes aware of any breach in the undertaking. In such an eventuality, you and the family members will have to leave Singapore within twenty-four hours of the cancellation.
You also need to understand that the Singapore Entry Visa Application form and your visa does not grant you entry into the country. It depends on the discretion of the officials at the points of entry.
While filling out Form 14 A, we had to undertake that the immigration department can verify and obtain relevant information from any source deemed fit for the assessment of our application. Thereon, we had to mention the date and sign the form.
Page 4 marks the end of the Singapore Entry Visa Application Form (Form 14 A). At the bottom corner, it states "IMM 14 A," followed by the page number "4" and the lines "This Form is Issued Free" below it.
Form 14 A is a straightforward form. Our advice is to ensure that the details on the form match the passport.
Some travelers have reported that their Singapore Entry Visa Application form was rejected on the grounds that they had shared their correspondence address instead of their permanent address.
Please note Form 14 A (entry visa application) differs from Singapore Form 14, a "Long Term Visit application form."
All incomplete Singapore Entry Visa Application forms will cause delays or may get rejected.
