Starting November 1, Costa Rica has opened flight entry for citizens from all countries across the world, provided they fulfil the country’s visa and COVID-19 entry conditions. For the safety of travelers and also its citizens, the Government of Costa Rica has implemented sanitary control guidelines for visitors, on arrival to the country. This online form is called "Pase de Salud" or "Health Pass", which is among such control measures. Travelers can get this form online and complete their Health Pass, following which they receive a QR Code. They need to display this code to the airport officials on reaching Costa Rica.
The Health Pass application form is completely online and simple to fill, conclude and pay using a credit card or via PayPal.
No embassy visit is required. Applicants receive their Health Pass on their email IDs within the comfort of their homes.

Travelers must fill out the Health Pass on their every travel to Costa Rica.
Passenger information is kept highly confidential as per the Costa Rican laws and regulations that protect a person’s right to privacy. This information is only available to the local Health and Immigration Authorities. The information submitted by applicants is used to prevent the dissemination of COVID-19 and for humanitarian benefits only.
The Costa Rican Ministry of Health is the government body that decides the use of this information. To know more about the information entered by applicants in their Health Pass, they can contact this department via email.
Filling this Health Pass form insinuates that the applicant is accountable for the factuality and precision of the data submitted, along with the approval of statutory requirements of data processing and compliance with all requirements defined by Costa Rican health authorities.
In the case of minors, the person’s legal guardian must fill their Health Pass. Something known as ’Guardian’s Information needs to be filled up, which only applies when the Health Pass form is being filled for a minor (below 18 years) or a dependent individual (aged or physically challenged individual, not in a position to fill out the form). This is the situation where you need to enter the information of this person’s legal guardian.
The requirements given continue throughout the pandemic:
1. Fill out the form known as HEALTH PASS and Submit it Online
The recipient receives the pass 48 hours before boarding. It is available online in an updated browser. Every applicant including minors, need to fill in a separate form.
2. Purchase Health Insurance
Travelers must have travel insurance that also covers their lodging when in quarantine and treatment costs owing to COVID-19 sickness. An interpretation of one’s travel insurance cover, meeting Costa Rica’s criteria, needs to be uploaded or filled into the HEALTH PASS for review and approval by the Costa Rican authorities. For international insurance, the tourist must request certification in English or Spanish from their insurer, with a minimum of three criteria, remembering to add it to the HEALTH PASS:
In case the travelers have no international insurance, they can choose travel insurance from Costa Rican companies approved by SUGESE (General Superintendence of Insurance of Costa Rica) that provide their services at Juan Santa Maria international airports (city of San Jose) and Daniel Oduber Quiros (city of Liberia).
National insurances fulfill every condition to have a medical expense cover for sickness, inclusive of COVID-19, of at least $20,000 and a minimum cover of $2,000 for additional accommodation expenses or quarantine, due to pandemic sickness.
Non-nationals with provisional or permanent residency or are temporary classified, who return to Costa Rica, and who are unaware of the corresponding payments to the Costa Rican Social Security Fund, need to purchase temporary regional insurance for 22 days or be internationally insured that is according to the requirements of an international tourist.
As far as foreigners with the migratory type of permanent or temporary residency status are concerned, or special class in the second situation, aside from Student Subcategory, need to display insurance by the Costa Rican social security to be verified.
Passengers of international private flights can also enter the country under the same conditions as mentioned earlier.
Travelers and crew of private maritime vessels and sailboats need to fulfil the same disinfection entry criteria that are mandatory for flight travelers to the country. Besides the requirements previously mentioned, passengers from yachts or sailboats also need to submit to the immigration officials, the document "International Ship Sailing and Dock" in authorized marinas.
